Privia Health is a wellness and population health management company that partners with top doctors to keep people healthy, and better manage disease. Position Summary
Privia Health Advisors consist of registered nurses, who along with registered dietitians, fitness trainers, and other top health professionals, serve as trusted advisors to Privia's members (the patient).  They provide phone based care management, wellness coaching, health education and health advocacy services. Personal Health Advisors help to coordinate care for our members and their primary care physicians, ensuring all parties have the right information they need to make the best clinical decisions. Personal Health Advisors support Privia members through developing an annual Health & Wellness Plan, providing targeted medical education or research, answering medical questions, reviewing and entering medical records in to a personal health record, and generally helping our members navigate the healthcare system so that they may receive the very best care and wellness support possible.

 

The relationship between a member and their Personal Health Advisor is a special one, as our members place a great deal of trust in our Advisors to help them with one of their most precious assets, their health. As such, Privia is looking for highly experienced health professionals who are committed to uncompromised excellence, exemplary customer service, and tangibly improving the lives of others. The ideal candidate demonstrates a desire to surprise and delight our members with above and beyond service and feels passionate about the field of health and wellness.

 

Core Responsibilities

  • Handling inbound communications from potential or current members who are seeking information; conduct outbound communications for follow-up and care coordination.
  • Consult and coordinate with internal and external team members to assess, plan, implement and evaluate patient care plans, make appropriate referrals, and provide follow-up.
  • Assist members with finding appropriate providers and care solutions and coordinate priority appointments.
  • Research information online and in Privia’s internal knowledge databases.
  • Provide health information, coaching, and critical thinking skills to assist our members with medical and wellness related issues.
  • Other duties as assigned to help members get better care and doctors provide better care.

Basic Qualifications

  • Current, unrestricted RN license in your home state
  • Minimum 3-5 years of recent clinical experience
  • A minimum of 2 years Disease Management, Case Management or Wellness experience is a must
  • Proven experience providing health coaching and/or helping members in pre and post-operative situations.
  • Experience working in a fast paced environment.
  • Without question, 'Exceptional Customer Service'
  • Strong computer skills.  Internet savvy.  
  • Clear, confident communication and listening skills
  • Self motivated and self disciplined
  • Strong entrepreneurial spirit.  A desire to work in a  highly rewarding, rapidly growing, environment
  • Willingness to do what it takes to get the job done and make members the number one priority
  • Problem-solving and decision-making skills
  • Able to thrive in a quiet, secure home office environment
  • Successfully pass credit, criminal and reference background checks
  • Detailed-oriented, organized with the ability to work well in fast-paced work environment
  • Privia will help set up the home office setting with computers, high speed internet access and other equipment needed for the job.

Additional Desired Qualifications

  • Bachelors degree or higher
  • Case Manager or Wellness Coaching certification
  • Geriatrics experience
  • Management or training experience
  • Bilingual - Spanish
